BECOME A CONFIRMED TIKOSYN INSTITUTION

To become a TIKOSYN institution, you must first complete the TIKOSYN Education Program. The objective of this program, developed by Pfizer, is to make TIKOSYN available only to prescribers and hospitals that have confirmed their participation in a designated TIKOSYN Education Program (referred to as "confirmed participants"). The program provides comprehensive education about the importance of in-hospital treatment initiation and individualized dosing.

A list of all confirmed participants of a TIKOSYN Education Program is maintained. The drug is available from mail-order pharmacies, and enrolled retail pharmacies. These pharmacies will verify prescribers and hospitals against this list before TIKOSYN is provided to patients or hospitals.

There are 2 ways to complete this program.

1. To request the TIKOSYN Monograph.
Please call 1-877-TIKOSYN to obtain a copy of the Monograph. Once you have studied the information, complete the Confirmed Education Institution Form and fax it to 1-800-788-2637. You can also mail the completed form to:

2. To view the Monograph through this Web site, click here. Once you have viewed the Monograph, complete the Confirmed Education Institution Form and submit.

On receipt of your "Confirmation of Education Form for Institution," your name will be added to the TIKOSYN institution list. Only then will your institution be authorized to receive the TIKOSYN product. Within 5 business days, you will receive a letter of confirmation.
